In order to receive replacements for Omega sprinklers and to qualify for installation cost reimbursement, the completed Proof of Claim Form and Waiver and Release of Claims Form must have already been sent by August 1, 1999, to Central Sprinkler, manufacturer of the Omega sprinkler heads. Once you have received the replacement sprinklers, you must also complete and return the Verification Form within 365 days to receive the reimbursement for installation costs.
Owners who file a Proof of Claim Form and Waiver and Release of Claims Form postmarked after August 1, 1999, and before November 1, 2001, are still qualified to receive free replacements, but will not be reimbursed for installation costs.
